I, Shirley Baker , do hereby certify that all facts, figures, and representations made in this application are true, correct, and consistent with the statement of assurances for these waivers. Furthermore, all applicable statutes, regulations, and procedures; administrative and programmatic requirements; and procedures for fiscal control and maintenance of records will be implemented to ensure proper accountability for the expenditure of funds on this project. All records necessary to substantiate these requirements will be available for review by appropriate state and federal staff.   • Use the findings of the parental involvement policy review to design strategies for more effective parental involvement, and to revise, if necessary, the school’s parental involvement policy [Section 1118(a)(E)];
  • If the plan for Title I, Part A, developed under Section 1112, is not satisfactory to the parents of participating children, the school will submit parent comments with the plan when the school submits the plan to the local educational agency [Section 1118(b)(4)];
  • Provide to each parent an individual student report about the performance of their child on the state assessment in at least mathematics, language arts, and reading [Section 1111(h)(6)(B)(i)];
  • Provide each parent timely notice when their child has been assigned or has been taught for four (4) or more consecutive weeks by a teacher who is not highly qualified within the meaning of the term in 34 CFR Section 200.56 [Section 1111(h)(6)(B)(ii)]; and

Involvement of Parents

Response: Everitt Middle School believes in involving parents in all aspects of its Title I programs. The SAC has the responsibility for providing input into the development, implementation and evaluation of the various school level plans, including the SIP and the PIP. SAC members are elected as voting members following SAC guidelines as directed by the State. All parents were given the opportunity to review the plans and to offer their input prior to approval. For the PIP, Spring surveys were sent to parents seeking their input on activities, training, and materials they needed to help their child. Results of the parent Spring survey were reviewed by the SAC to determine needed changes. During SAC meetings when PIP and SIP are discussed, the committee will decide, with input from parents how the parental involvement funds will be used. Marlo Munn Parent Liasion is responsible to maintain documentation from parents.

Flexible Parent Meetings



Describe how the school will offer a flexible number of meetings, such as meetings in the morning or evening, and may provide with Title I funds, transportation, child care, or home visits, as such services related to parental involvement [Section 1118(c)(2)].

Response: The leadership and staff of Everitt Middle School have a strong belief in the importance of parental involvement and so offer meetings at a variety of times to accommodate the schedules of parents. Parent survey data indicated this was a need and survey data was used to help determine the most convenient times for parents to attend meetings. SAC meetings will be held in the early evening, after school hours. Workshops and other parent meetings will be scheduled during the evening hours. In an effort to offer access to information to parents who cannot attend any of these meetings, Everitt is upgrading its school website to include information and handouts from these meetings. Additionally, the school newsletter will contain information about the meetings and hard copies of handouts will be available in the school front office in the Title I notebook. Everitt is upgrading website and will be available soon for parents to use.

Response: A parent involvement committee consisting of teachers and parents will meet regularly to investigate and plan parent involvement at Everitt. Additionally, available throughout the year a Title I Notebook of important information for parents is located in the front office so that parents who cannot attend meetings have information available to them. The Parent Liasion is responsible for maintaining and updating this. Information is also available to parents on both the school and district websites and the parent portal. Teachers will update information on the parent portal for parents and the administrator will ensure websites are maintained. Informational brochures are available to parents throughout the year in the guidance office. Newsletters will contain information encouraging the parents to attend SAC and other meetings as well as tips for working with their students at home. Jessica Brantley produces newsletters for parents at least 7 times during the school year.

Response: At the annual meeting of parents in September, Everitt Middle School will hold a general meeting where information will be presented about the Title I program, curriculum and academic assessment. Parents will be given an overview of the PIP and other information including websites and ways to assist their child at home. During classroom visits, teachers will provide additional information relating to their classroom and subject matter. Teachers will maintain sign-in sheets and provide a copy to the Principal for documentation. The Title I notebook and log in the front office will be maintained to document when parents requested and received information. Informational packets for parents who were unable to attend the annual meeting will be available in the school office. Additionally, throughout the year, parents receive information through a variety of media such as the school newsletter; school and district websites; teacher communication; distribution of report cards, and other assessment data; and through a variety of parent workshops and parent conferences. Response: Everitt Middle School will take the necessary steps to ensure that communications for all parents are in a format and to the extent practical, in a language parents can understand. Materials will be translated when feasible and/or requested by parents to ensure all parents have the opportunity to participate in the education of their child. Should interpreters be needed at workshops or meetings, one will be provided to the extent practical and for a language parents can understand. District employs a Spanish speaking parent liaison who is available to translate and one of the Everitt guidance counselors is bilingual in Spanish. Title I offers additional translation services. Spanish,Vietnamese,Russia and Chinese are predominant requests for translations, although other languages are available. Sign language is also available. All buildings are handicap accessible. All parents may request auxiliary aids and services of their choice such as sign language, Braille, etc. to ensure meaningful participation by informing the school of this need when enrolling a student or by phone call, e-mail or letter to the AP or Principal. Additionally, parents can request services on any workshop evaluation form. All requests for services will be kept by the Principal and/or Title I Parent Liasion for documentation purposes.

Everitt will continue to offer parent/family events in the evening. We will make the information from meeting available to parents in a hard copy format.
